I hope this letter finds you well in these challenging times.

We are writing to you today to inform you that six physicians from the ByWard FHT have decided to relocate their practices to Santé Alliance Health, as of September 1, 2021.

The permanent address of this new 5000 sq foot facility, designed for optimal patient care, will be located at:

We anticipate that construction at our new facility will be completed in the spring of 2022.  In the meantime (and as of September 1, 2021), Santé Alliance Health will be located at:

 We invite all our OHIP patients (from the above-mentioned physicians) to join us.  If booking an appointment after August 16, please call the clinic at 613-801-3380.  Be advised that a copy of your electronic patient record will be automatically transferred to Santé Alliance Health. Also, please feel free to refer family and friends to our team, as several new family physicians will be establishing their practices. They can visit our “new patient registration” section on our website:  https://www.alliancehealthohc.ca    to register with a family physician.

If seeking medical care elsewhere, your written authorization is required to release your medical record.  Prior to September 1, 2021, contact ByWard FHT directly, via fax or mail, to send a copy of your medical record to another facility. An administrative fee for this uninsured service will apply.  A consent to release personal health information can be found on the ByWard FHT website at: https://bywardfht.ca/our-clinic/forms/ . If a request is made after September 1, 2021, patients are to go to the Santé Alliance Health website at https://alliancehealthohc.ca.

If you need to find a new family doctor, you can register with Healthcare Connect via their website https://ontario.ca/healthcare connect or register via the College of Physicians and Surgeons of Ontario. On their website, you will find a link for “CPSO doctor search”. You can then find a doctor by selecting “advanced search” to narrow by region or “additional search” to narrow your query. Once this is done, you can call the doctor’s office to ask if they are accepting new patients.

Note:

Due to limited capacity, the ByWard FHT will not be able to accommodate patients who wish to register with another provider at the ByWard FHT, with the exception of uOttawa and uStPaul students and staff. Students and staff may contact the ByWard FHT at 613-564-3950 and request that their name be put on waiting list to be registered to a new primary care provider. While ByWard FHT is doing what they can to accommodate patients, adding your name to the waitlist does not guarantee a primary care provider. Walk-In services will remain an option for uOttawa and uStPaul students and staff only.
