ByWard Family Health Team Patients
We invite all our OHIP patients currently registered with Dr. Annemarie Allart, Dr. Adnan Alrai, Dr. Louise Bourgault, Dr. Milka Gostimirovic, Dr. Paula Tchen and Dr. Meena Sharma at ByWard FHT to join us at Santé Alliance Health.
A copy of the electronic medical records of patients from ByWard FHT will be automatically transferred to Santé Alliance Health.
For patients who do not wish to transfer their care to Alliance Health, please fill this form at ByWard FHT website and contact ByWard FHT by fax or mail before 1 Sep, 2021. After 1 Sep, 2021, please contact Santé Alliance Health to transfer your medical records to another provider.

Health Treasure Medical Centre Patients
Dr. Adnan Alrai is inviting all the patients currently registered with him at the Health Treasure Medical Centre to join him at Santé Alliance Health.
The patients at Health Treasure Medical Centre will need to contact Health Treasure Medical Centre to obtain a copy of their medical record. Health Treasure Medical Centre may charge a fee for this uninsured service.
Friends and family members of Dr. Alrai’s patients who wish to register with Dr. Alrai can fill in this new patients registration form and select Dr. Alrai as the preferred family physician.
Bells Corner Medical Group Patients
Dr.Gail White at Bells Corner Medical Group has made arrangements to transfer care of her patients to Dr. Stanislav Terziev at Santé Alliance Health.
A copy of the electronic medical records of Dr. White’s patients from Bells Corner Medical Group will be automatically transferred to Santé Alliance Health.
For patients who wish to transfer their care and electronic medical records to a different health care provider, please contact Bells Corner Medical Group. A charge for this uninsured service may be applicable.

Some of our physicians are building new practices and accepting new patients. Please fill this new patient registration form if you’d like to join our waiting list. Our staff will start contacting patients on a first come, first serve basis.
